Licensed Clinical Social Worker (LCSW) is responsible for performing thorough and accurate psychosocial evaluations, developing and implementing master treatment plans, and providing for appropriate discharge planning/aftercare referrals. The Clinician/Therapist, Counselor demonstrates the ability to effectively communicate with both clients and staff. LCSW provides quality, competent, safe and compassionate care based on established standards in a patient-focused preventative and treatment care. LCSW must have strong knowledge of the DSM –V, including a strong understanding of psychosis and OCD. Recovery oriented and trauma informed. LSCW is well versed in Motivational Interviewing techniques, Cognitive Behavioral Therapy, and Behavioral Activation. LCSW must have solid understanding DSM, CBT and motivational interviewing skills. Position require well-grounded in mental illness and substance use to provide therapy with serious mental illness, personality disorders, depression, anxiety disorders and substance abuse and/or triage and must know how to make a psychiatric diagnosis.

RESPONSIBLITIES AND DUTIES:

Duties and Responsibilities include but not limited to:

Provides comprehensive assessment, conceptualization and diagnosis using the DSM-V. Develops treatment plans and aftercare planning.

Provides treatment services to a caseload of 30-50 clients, the caseload number being based on the following factors: show rate, modalities provided and maintaining weekly utilization/billing targets.

Prepares ahead for sessions, tailoring treatment to each patient’s unique needs.

Comfortable confronting, contracting with, and making clients accountable for their own choices.

Involves clients in their own treatment planning as well as their own education about their mental illness(es)

Provides cases intervention, ensure clients admitted into emergency room, assisting in the development of plan for clients at risk for suicide/homicide. Follow-up on emergency cases as needed

  • Develops and maintains clinical records, in accordance to funders standards
  • Provides individual and group therapy in an outpatient setting, via telemedicine and/or in-person treatment.
  • Attends and contributes to team meetings and peer supervision.
  • Communicates and collaborates regularly with patient care team–both other mental health care provider and other medical providers.
  • Provide instructions to the patient upon discharge

Experience Requirements:

Master’s Degree in Social Work

Licensed Clinical Social Worker (LCSW) by the State of New York

Experience with Evidence Based Practices.

Experience working in an Article 31, Article 28 and/or an FQHC a plus.

Bilingual Spanish strongly preferred.

Experience working with individuals and families affected by mental illness.

Job Type: Full-time

Salary: $70,000.00 – $80,000.00 per year
